Notes:

  1. On Standard Database Format B or C, only use a PO Box or Street Number in an address field. Do Not include both.
  2. In the Zip Code field, include the leading zeros in the first 5 digit of the Zip Code if there are less 5 digits (ie 00245).
  3. For a 5+4 Zip Code use the following format: 32903-1134, 00234-1245
  4. If you want all the address fields from your database to appear with mixed upper and lower case characters on the mailing labels, you will have to communicate that information to Address N’ Mail. Our standard format is to use all upper case characters for the mailing labels.
  5. Make sure that the database you send us has the First Name first in the Full Name Field.
  6. If there is a suite number for Format A and C, make sure you include it in the address field.
  7. For Format B and D make sure you include the suite number and/or mail stop in one of the address fields.
  8. If you wish to include the title of the individual, include that in the Full Name field.
  9. To prevent additional database service charges, please make sure we receive you information in one of the above formats.
